In 【閲覧注意】実録！ほんとにあった呪いの神経衰弱【最恐ホラー】 ([Warning] True story! Real Cursed Nervous Breakdown [The Scariest Horror], players are turning cards and looking for matching images. If they find a matching image, they take the pair of cards out of the game. But if they find an unmatched pair, they draw a new card, look at it in secret, and place it face down with the rest.

However, there are two cards showing a weeping doll which is cursed. If a player turns up the cursed doll, all the players must immediately use their protective talisman, a votive prayer sheet, by slamming it down on the doll. The last player to do this receives a candle, meaning they are one step closer to death. If another player is then too slow, the candle moves on. But if the player with the candle fails a second time, they are killed by the curse and eliminated from the game. There is a terrible demon card too, which immediately kills the player with the candle.

All the cards depict very disturbing photographs of normal locations such as a bathroom or a cable car, but showing ghosts in the mirror, or shadowy figures, or bloody handprints. Further, some cards carry penalty instructions, such as requiring the player to cover one eye, making it harder for them to react when the cursed doll appears.

The last player to survive the increasing horror is the winner.
